This page lists our accepted insurances, health plans, and forms of payment.

PPO and POS:


  • Aetna

HMO (Requires Primary Care Physician authorization):

  • Inland Valley IPA/Synermed
  • Regal Medical Group
  • TriCare Prime (Authorization required, or else services are billed at POS benefit rates)

Medicare and Medi-Cal:

  • Medicare Part B (Existing patients only; we are not accepting new Medi-Care patients)
  • Medi-Cal (Existing patients only; we are not accepting new Medi-Cal patients)

Medicare Part B HMO:

  • Regal Medical Group



If you don’t see your particular health plan listed, or if you have any other insurance-related questions, please contact our office to verify coverage.

Self-Pay:

If we are not contracted with your insurance plan, or if you do not have insurance, please call our office for more information on our Self-Pay cash discounts. If you have a PPO plan that we are Out of Network for, we can bill them to utilize your Out of Network benefits, but we will still collect Self-Pay rates at the time of service. We accept cash, personal check, debit, and credit cards (MasterCard, Visa, Discover, and American Express). We are also familiar with Health Care Sharing Ministries such as Samaritan Ministries, and we can provide you with any information you may need in order to help you process your Medical Need.
